These are additional items that you will need to bring to your appointment:
- Insurance Card
- Driver’s License
- Written Referral from PCP (for HMO plans)
- Medication List
- Medical records from referring physician
- Breast Patients: Mammogram/Sonogram Films
Patients under the age of 18 must be accompanied by a parent or legal guardian.
